To secure an appointment, three things are needed

1.  A call, to check whether suitable help can be provided, and the provision is suitable to you.  A provisional appointment can be arranged at the end of the call. 

2. Return of the completed Terms of Service, and Patient details forms

3. A payment plan - either a deposit of £50 for self pay, or pre authorisation if via insurance

Once completed, the appointment date and timing will be confirmed
